This ministry is supported with generous financial donations that are used to help keep us functioning. Our purpose is to help supply individuals and families with personal needs such as food, toiletries, clothing, household items, furniture and baby/adult diapers/wipes both locally and to areas effected by disasters . At this time, we do not offer any shelter or transitional housing services. Again, we DO NOT pay any bills. We DO NOT pay any rents or mortgages.

1. DIAL “211” from a land-line telephone. This will not work from a cell telephone. Tell the receiver your situation and what you need. They will give you information specific for the location you are currently in plus any referrals you may need. They have the most up to the minute information for your specific area. Dialing “211” will get you help and much more info than this office can provide. Homeless to Independence wants you to have all the information you need to get through the situation you are in.

Don’t have a land-line telephone??? Go to your local police department, library, or house of worship and ask to use their land-line telephone. They may even make the call on your behalf.

DRUG & ALCOHOL ADDICTION RECOVERY PROGRAM:

Faith Farm Ministries is a free, 9 month, faith-based, addiction recovery program with residential services for
more than 400 men and women who have lost control of their lives due to alcohol and/or illegal and
pharmaceutical drug addictions.

FIGHTING ADDICTION… IT IS EVERYONE’S BATTLE! This has become the Faith Farm Ministries Motto!
Everyone knows someone … has loved ones, friends, siblings, parents, children, co-workers and/or neighbors
that suffer from addiction. Addiction is an epidemic!

Because of special people like you, Faith Farm continues to serve the ever-growing need for our services in
our community at no cost to our students. Faith Farm has also become a viable alternative to incarceration,
saving the citizens (including you) thousands in tax dollars. It is estimated that over 37,000 people have been
helped in Faith Farm’s effective recovery program.

Our comprehensive program is designed to help the whole individual to heal physically, psychologically and
spiritually. We prepare our students for living full and productive lives as they return to their families and their

Individuals and families face the difficulties associated with homelessness each and every day here in Manatee
County. Our Transitional Development Program helps people with budgeting so they can understand the limits
of their situation, and assists with financial payments for first month’s rent and/or security and utility deposits
as funding allows.

To begin the process of receiving assistance, people need to call our office at 747-1509. You will be asked a
series of questions regarding your situation, and based on your answers, you will know whether you meet
basic program eligibility requirements. If eligible, you will then be scheduled for an appointment with the Case
Manager.

The Case Manager conducts a thorough interview with each client, assessing their need and eligibility for
different funding sources that are available to help with rent or deposits. If you are seeking financial rental
assistance you must meet eligibility requirements. A primary requirement is proof of income to maintain
yourself in housing for a minimum of six months. You must also be able to prove residency in Manatee County
for at least thirty (30) days.

Our Daily Bread

Meals served daily between the hours of 10:00 am to 11:30 am.

Our Daily Bread is a soup kitchen that provides meals to the homeless and struggling families in the Bradenton
community.

We have recently moved into the One Stop Center (located at 701 17th Avenue West), and are open 364 days a
year (only closing on Thanksgiving). We turn no one away. The only requirement is that you show up and
come hungry. Our volunteers make all of our meals here in our kitchen. Each meal contains 1200-1800
calories. We serve high calorie, well-balanced meals to the homeless to provide them with enough calories for
the day in case they are unable to find another meal for the day.

Beyond just feeding the hungry:

Aside from just feeding the hungry we provide family and homeless bags. Our family bags are passed out to
struggling families in need and our homeless bags are passed out to the struggling homeless. Each bag
contains simple essential supplies.

This letter of introduction for Transitional Resources Incorporated is to explain our agenda, which is currently
in place. Transitional Resources Incorporated began operating in July of 2002 with our first sober home for
men in Manatee County. Since opening those first doors, Transitions has expanded services by opening homes
for women, The TRI Center and into Sarasota County. Transitions have created a solid reputation for
maintaining and operating a safe secure home environment for individuals with substance abuse addiction.

Each client, upon admittance, is interviewed and screened in order to obtain their recovery goals. It is
essential that new clients show the potential and desire to want to change. In reviewing the client intake
information Transitions clearly states what is expected of each client relative to house rules and procedures.
Transitions philosophy is self- governance, requiring responsibility and accountability with recovery and
communal living. Each residence has a live in full time manager on the premises. The manager’s primary
function is to supervise the care for the home, assure client compliance in getting along with others, assign
and follow up on client chores and to assist and support clients as needed.

With the success of our first house, a second home was established in November of 2003, for women,
operating under the same guidelines and requirements as the men’s residence. The manager selected has
been sober and clean for several years and is well respected in the recovery community. This house is where
eight (8) women, very proudly, call home. They are diligent in their chores and are proud to show it off at any
given time. 12 Step meetings are regularly held throughout the week.

In October of 2007 Transitional Resources Incorporated contracted with Sarasota County as the transitional
housing provider for the Community Alternative Residential Treatment Initiative (CART) program. The
opportunity to serve Sarasota has been very rewarding, both for Transitions and the recovering community.
Currently, we operate two men’s houses and one woman’s home.

On September 1st, 2009 Transitions was given the opportunity to enter into partnership with Manatee Glens,
the area’s oldest and largest not-for-profit provider of mental health and addiction treatment services. This
joint venture resulted in the opening of the TRI Center located at 1819 5th Street West in Bradenton, Fl. The
TRI Center accommodates eight (8) women and ten (10) men. The Center has a full kitchen, dining room, TV
room and laundry. Outside features a basketball court, volleyball court and designated smoking area. A small
pond offers the perfect setting for peace and quiet for meditation and reflection. The Center ground also has a
separate building that will be used for 12 Step meetings and a computer lab.

We presently offer our services to interested clients for a weekly fee. Clients are encouraged to buy and
prepare their meals. Our well maintained and supplied kitchens offer everything anyone would need to make
their food just the way they want it. Transitions provide bed linens, pillows, blankets and bath towels. Each
home has a washer and dryer for the client’s convenience.

We would appreciate the opportunity to meet with the appropriate staff to discuss providing transitional

What does the daily schedule consist of? Daily Schedule

– When are residents permitted to leave the ministry property?

As a group or with an assigned Staff.

– What do you do on weekends?

Saturday we spend about half the day working on the property, the balance of the time is broken up between
some recreation, showers, and free time and an evening chapel. Sunday we visit churches and share our
testimonies.

– Are Residents allowed mail?

A Resident may request to write immediate family after he has been here for 30 days, once approved he may
write. A Resident may then receive mail from the approved family. All mail is reviewed by staff.

– Are Residents allowed phone calls?

A Resident may request to call his immediate family after he has satisfactory completed 60 days in the
program, once approved he is allowed one 10 minute supervised call on Sundays.

– Are Residents Allowed Visits?

A Resident may request a visit with his immediate family after satisfactory completing 2 weeks in the
Sophomore Phase. Visits are 2-4 hours on property with supervision on weekends.

– Care Packages?

Sender must seek approval from Staff before sending.
