Program Description
The Medicaid Program provides medical benefits to low income people who have no medical insurance or have inadequate medical insurance. The Federal government establishes general guidelines for the administration of Medicaid benefits.
The Arizona Medical Assistance Program provides medical services that are approved by Arizona Health Care Cost Containment System (AHCCCS).
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the state of Arizona, a U.S. national, citizen or permanent resident in need of health care/insurance assistance, whose financial situation would be characterized as low income or very low income.
Arizona Medicaid programs provide coverage for state residents who meet the general program requirements and meet income requirements. However, enrollment for childless adults who are not aged (65+), blind or disabled is frozen as of July 8, 2011. Medicaid programs for children, pregnant women, and the aged, blind or disabled are not affected by the enrollment freeze.
In order to qualify you must also have an annual household income (before taxes) that is below the following amounts:

Arizona Cash Assistance Program

Program Description
The Arizona Cash Assistance Program provides temporary cash assistance and supportive services to children, individuals, and their families. The Electronic Benefit Transfer (EBT) EDGE System is used to issue cash assistance payments. Participants are able to access their benefits from their assigned account by using a plastic debit card called QUEST. The debit card can be used at Automated Teller Machines (ATM) and Point of Sale (POS) devices located at most stores.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the state of Arizona, responsible for a child under 19 years of age, a U.S. national, citizen, legal alien, or permanent resident, have low or very low income, and be either under-employed (working for very low wages), unemployed or about to become unemployed.

Program Description
KidsCare is Arizona’s health insurance for children under 19.
Children ages 18 and younger that qualify can get medical, dental and vision services; all three services combined in one simple plan.
KidsCare is inexpensive; a family’s payments are based on the family income and the number of children who qualify. KidsCare will
cost no more than $50 a month for one child or no more than $70 a month no matter how many children are in the household. Some
families may even receive KidsCare at no cost.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the state of Arizona, under 19 years of age, not covered by
health insurance (including Medicaid), a U.S. National, citizen, legal alien, or permanent resident, and you must have an annual
household income before taxes of less than $29,140 if two people live in the household; $36,620 if three people live in the household;
$44,100 if four people live in the household; $51,580 if five people live in the household; $59,060 if six people live in the household;
$66,540 if seven people live in the household; $74,020 if eight people live in the household; and $81,500 if more than eight people live
in the household. For larger households, add $7,480 for each additional person in the home. Depending on your income level, you may

Applying for KidsCare is simple and easy and can be done by mail. There are no interviews, so no one has to miss work. To download

NOTE: KidsCare Program Changes. The KidsCare Office is unable to approve any new applications. Enrollment in the KidsCare
Program has been frozen since January 1, 2010 due to lack of funding for the program, however, you can still apply. DES will review
your application first, to determine if your family may be eligible for AHCCCS Health Insurance. If your children are not eligible for
AHCCCS Health Insurance, but it appears they may be eligible for KidsCare, and you are willing to pay a premium, DES will send their
information to the KidsCare Office to add them to the KidsCare waiting list. The waiting list will be prioritized based on the date of the
application. The oldest application date will be on the top of the list. If funding becomes available, the KidsCare Office will contact you.
The KidsCare Office is processing renewals and changes for eligible children. Families with eligible KidsCare children must complete
their renewal and make their premiums on time to avoid losing KidsCare coverage.

Program Description
Utility assistance programs available from the Federal and state governments and from utility companies are coordinated to help low-
income households meet their energy needs. The Community Services Administration (CSA) subcontracts with community agencies
to administer the Low-Income Home Energy Assistance Program (LIHEAP) and the Utility Repair, Replacement and Deposit Program
(URRD). Through these programs services provided are utility deposits and payments. Weatherization of homes is conducted and
certain appliances can be repaired or replaced. There are also wide arrays of utility discount programs, which are coordinated with
these services.

The Weatherization Assistance Program was established to help low-income families and individuals decrease their home energy
costs and to be attentive to energy-related health and safety issues in the home. The Arizona Department of Commerce Energy
Office is the managing agency for the Weatherization Assistance Program. More than 16,500 of Arizona’s low-income households
have been provided with weatherization assistance services since the program’s inception in 1977. Arizona’s current annual funding
allocation is approximately $2.5 million.

Program Description
While learning does not end when school lets out, neither does the need for good nutrition. The Summer Food Service Program
(SFSP) was created to ensure that children in low-income areas could continue to receive nutritious meals during long school
vacations (i.e. school intercessions and summer vacation), when they do not have access to school breakfast or lunch. Without the
help of the SFSP, many children are hungry and undernourished when school is not in session.
SFSP is Federally funded by the United States Department of Agriculture (USDA) and administered by the Arizona Department of
Education (ADE). Sponsors of SFSP must sign an agreement with ADE in order to run the program. The SFSP reimburses approved
sponsors for serving meals that meet Federal nutritional guidelines. Sponsors receive payments based on the number of meals they
serve. All meals are served free to eligible children.

The WIC Program provides supplemental foods, nutrition education and referrals to health care, at no cost, to low-income pregnant,
breastfeeding and postpartum women, infants, and children up to age 5 who are determined to be at nutritional risk. To be fully

certain medical-based or diet-based risk conditions).

Program Description
The Special Milk Program (SMP) is available to children of all schools or nonprofit child care institutions which do not participate in
the National School Lunch Program (NSLP), School Breakfast Program (SBP), the Child and Adult Care Food Program (CACFP), or
Summer Food Service Program (SFSP). This includes nonprofit day care centers, summer camps, settlement houses and homeless
shelters. An eligible shelter’s primary purpose must be to provide shelter and a minimum of one regularly scheduled meal service
per day to homeless families. The shelter cannot be a residential child care institution.
The SMP is also available to students attending a split-session kindergarten or pre-primary students in a school that participates in
the NSL or SBP, if those students do not have access to the lunch or breakfast programs.

Any child in a school or institution that participates in the Special Milk Program can get milk. Schools may elect to offer free milk to

Program Description
The Arizona Head Start program (for children ages 3-5) and Early Head Start program (for pregnant women, infants, and toddlers)
promote school readiness for children in low-income families by providing comprehensive educational, health, nutritional, and social
services. Parents play a large role in the programs, both as primary educators of their children and as participants in administering
the programs locally. Both programs provide pre-literacy and literacy experiences in a multi-cultural environment. Parents are also
provided social services, including assistance with child care. Services are also available to migrant and seasonal farm worker
families.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the State of Arizona, you must be a parent or primary caregiver
responsible for a child who is too young for public school and your household’s annual income before taxes must not exceed $10,830
if one person lives in the household; $14,570 if two people live in the household; $18,310 if three people live in the household; $22,050
if four people live in the household; $25,790 if five people live in the household; $29,530 if six people live in the household; $33,270 if
seven people live in the household; $37,010 if eight people live in the household; and $40,750 if more than eight people live in the

If you do not meet these criteria, you may still qualify if you are a U.S. National, citizen or permanent resident whose financial status

The Southern Arizona Chapter serves Pima, Cochise, Graham, Greenlee and Santa Cruz counties.
