Program Description
New Hampshire Healthy Kids provides access to low-cost and free health coverage options for New Hampshire's uninsured
children and teens.
General Program Requirements
Your child or teen may be able to enroll in a Healthy Kids plan if he/she is:
•Under the age of 19
•A New Hampshire resident
•A U.S. citizen
•A refugee, in the U.S. for at least five years
Family Income
The Healthy Kids programs were designed for working families. Your family income and size will determine if your child qualifies for
coverage and if you are required to make monthly premium payments. Visit http://www.nhhealthykids.com to see the current
income guidelines chart.
Current or Recent Insurance
Even if your child is currently insured, he or she may still be able to enroll in Healthy Kids Gold and receive services not covered by
your private insurance. Healthy Kids Gold may also pay for certain medical services your child received up to 90 days before you
signed up.
To qualify for Healthy Kids Silver, your child must have been uninsured for the past six months. This requirement may not apply in
special situations such as job loss.

Program Description
The Financial Assistance for Needy Families Program (FANF) provides cash assistance to families with dependent children through
either the Family Assistance Program (FAP), the New Hampshire Employment Program (NHEP), the Unemployed Parent program, or
the Interim Disabled Parent program. To qualify, the dependent children must lack parental support or care due to death, continued
absence or because one or both parents in a two-parent home are disabled, unemployed or working less than 100 hours a month.
Parents of families eligible for FANF cash assistance are also eligible for Medicaid; children of FANF eligible families are eligible for
Healthy Kids Gold medical coverage. Families eligible for FANF who do not live in government subsidized housing are also eligible
for Emergency Assistance that can help them get and keep safe and secure housing.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of New Hampshire responsible for a child under 20 years of age,
a U.S. national, citizen, legal alien, or permanent resident, have low or very low income, and be either under-employed (working for

pregnant, breastfeeding and postpartum women, infants, and children up to age 5 who are determined to be at nutritional risk. To
be fully eligible for the WIC Program, applicants must be determined by a health professional, at no cost, to be at nutritional risk (i.
e., have certain medical-based or diet-based risk conditions).

In order to qualify for this benefit program, you must be a resident of the State of New Hampshire, pregnant, breastfeeding and
postpartum woman, infant or child up to 5 years of age and: (1) are individually determined by a health professional to be at nutrition

Program Description
The New Hampshire Medicaid program is a Federal and state funded program that serves needy individuals and families in New
Hampshire who meet financial and other eligibility requirements and certain other individuals who lack adequate resources to pay
for medical care.
The program provides payment for medical services ranging from routine preventive medical care for children to institutional care
for the elderly and disabled. The Medicaid program also ensures access to care through enrollment of medical providers and
access to transportation services to medical care providers through reimbursement to recipients and volunteer drivers.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the state of New Hampshire, a U.S. national, citizen, permanent
resident, or legal alien, in need of health care/insurance assistance, whose financial situation would be characterized as low
income or very low income. You must also be either pregnant, a parent or relative caretaker of a dependent child(ren) under age 19,
blind, have a disability or a family member in your household with a disability, or be 65 years of age or older.

The New Hampshire Office of Energy and Planning Fuel Assistance Program (FAP) may be able to provide some relief for residents
who are having trouble paying for heat during the winter season. The Office of Energy and Planning (OEP) contracts with six local
Community Action Agencies (also known as CAAs) to provide service to eligible households. As funds become available, the Fuel
Assistance Program provides benefits to qualified New Hampshire households to assist with heating costs. Renters and
homeowners are eligible for the Fuel Assistance Program.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the state of New Hampshire , you must need financial
assistance for home energy costs and your household's annual income before taxes must not exceed $27,651 if one person lives
in the household; $36,159 if two people live in the household; $44,667 if three people live in the household; $53,175 if four people live
in the household; $61,683 if five people live in the household; $70,191 if six people live in the household; $71,786 if seven people live
in the household; and $73,382 if eight people live in the household. For larger households, add $1,596 for each additional person in
the home.

The Food Stamp Program is about good nutrition and health. It provides eligible individuals and households with benefits used to
purchase food items at grocery stores, supermarkets or other participating food retailers. The Food Stamp Program ensures that
all recipients have an opportunity to learn more about purchasing and preparing nutritious meals.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the State of New Hampshire and fall into one of two groups: (1)
those with a current bank balance (savings and checking combined) under $2,001, or (2) those with a current bank balance
(savings and checking combined) under $3,001 who share their household with a person or persons age 60 and over, or with a
person with a disability (a child, your spouse, a parent, or yourself). For either group, you must also have an annual household

The Special Milk Program (SMP) was established to encourage consumption of fluid milk by children in public schools, child care
centers, after-school-hours child care programs, summer camps, and similar non-profit institutions devoted to the care and
training of children. The SMP is part of the Bureau of Nutrition Programs and Services, which provides educational leadership and
services that promote health and nutrition education for all citizens through quality nutrition programs in New Hampshire schools
and institutions.

The Head Start program (for children ages 3-5) and Early Head Start program (for pregnant women, infants, and toddlers) promote
school readiness for children in low-income families by providing comprehensive educational, health, nutritional, and social
services. Parents play a large role in the programs, both as primary educators of their children and as participants in administering
the programs locally. Both programs provide pre-literacy and literacy experiences in a multi-cultural environment. Parents are also
provided social services, including assistance with childcare. Services are also available to migrant and seasonal farm worker
families.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the State of New Hampshire, you must be a parent or primary
caregiver responsible for a child who is too young for public school and your household's annual income before taxes must not
exceed $10,830 if one person lives in the household; $14,570 if two people live in the household; $18,310 if three people live in the

household; $33,270 if seven people live in the household; $37,010 if eight people live in the household; and $40,750 if more than
eight people live in the household. For larger households, add $3,740 for each additional person in the home.
If you do not meet these criteria, you may still qualify if you are a U.S. national, citizen or permanent resident whose financial status
is low income or very low income, who is under-employed, unemployed or about to become unemployed, facing pregnancy, less

Program Description
The New Hampshire Medicaid program is a Federal and state funded program that serves needy individuals and families in New
Hampshire who meet financial and other eligibility requirements and certain other individuals who lack adequate resources to pay
for medical care.
The program provides payment for medical services ranging from routine preventive medical care for children to institutional care
for the elderly and disabled. The Medicaid program also ensures access to care through enrollment of medical providers and
access to transportation services to medical care providers through reimbursement to recipients and volunteer drivers.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the state of New Hampshire, a U.S. national, citizen, permanent
resident, or legal alien, in need of health care/insurance assistance, whose financial situation would be characterized as low
income or very low income. You must also be either pregnant, a parent or relative caretaker of a dependent child(ren) under age 19,
blind, have a disability or a family member in your household with a disability, or be 65 years of age or older.

The New Hampshire Office of Energy and Planning Fuel Assistance Program (FAP) may be able to provide some relief for residents
who are having trouble paying for heat during the winter season. The Office of Energy and Planning (OEP) contracts with six local
Community Action Agencies (also known as CAAs) to provide service to eligible households. As funds become available, the Fuel
Assistance Program provides benefits to qualified New Hampshire households to assist with heating costs. Renters and
homeowners are eligible for the Fuel Assistance Program.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the state of New Hampshire , you must need financial
assistance for home energy costs and your household's annual income before taxes must not exceed $27,651 if one person lives
in the household; $36,159 if two people live in the household; $44,667 if three people live in the household; $53,175 if four people live
in the household; $61,683 if five people live in the household; $70,191 if six people live in the household; $71,786 if seven people live
in the household; and $73,382 if eight people live in the household. For larger households, add $1,596 for each additional person in
the home.

The Food Stamp Program is about good nutrition and health. It provides eligible individuals and households with benefits used to
purchase food items at grocery stores, supermarkets or other participating food retailers. The Food Stamp Program ensures that
all recipients have an opportunity to learn more about purchasing and preparing nutritious meals.

In order to qualify for this benefit program, you must be a resident of the State of New Hampshire and fall into one of two groups: (1)
those with a current bank balance (savings and checking combined) under $2,001, or (2) those with a current bank balance
(savings and checking combined) under $3,001 who share their household with a person or persons age 60 and over, or with a
person with a disability (a child, your spouse, a parent, or yourself). For either group, you must also have an annual household

The Special Milk Program (SMP) was established to encourage consumption of fluid milk by children in public schools, child care

The Head Start program (for children ages 3-5) and Early Head Start program (for pregnant women, infants, and toddlers) promote

the programs locally. Both programs provide pre-literacy and literacy experiences in a multi-cultural environment. Parents are also
provided social services, including assistance with childcare. Services are also available to migrant and seasonal farm worker
families.

In order to qualify for this benefit program, you must be a resident of the State of New Hampshire, you must be a parent or primary
caregiver responsible for a child who is too young for public school and your household's annual income before taxes must not
exceed $10,830 if one person lives in the household; $14,570 if two people live in the household; $18,310 if three people live in the
household; $22,050 if four people live in the household; $25,790 if five people live in the household; $29,530 if six people live in the
household; $33,270 if seven people live in the household; $37,010 if eight people live in the household; and $40,750 if more than
eight people live in the household. For larger households, add $3,740 for each additional person in the home.
If you do not meet these criteria, you may still qualify if you are a U.S. national, citizen or permanent resident whose financial status
is low income or very low income, who is under-employed, unemployed or about to become unemployed, facing pregnancy, less
