Program Description
The Weatherization Assistance Program is the nation's largest residential energy efficiency program. Its mission is to insulate the
dwellings of low-income persons, particularly the elderly, persons with disabilities, families with children, high residential energy
users, and households with a high energy burden, in order to conserve needed energy and to aid those persons least able to afford
higher utility costs.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the State of Louisiana, you must need financial assistance for
home energy costs and your household's annual income before taxes must not exceed $18,768 if one person lives in the
household; $24,540 if two people live in the household; $30,324 if three people live in the household; $36,096 if four people live in the
household; $41,868 if five people live in the household; $47,652 if six people live in the household; $48,732 if seven people live in the
household; and $48,912 if eight people live in the household. For larger households, add $1,008 for each additional person in the
home.

Program Description
The Food Stamp Program provides monthly benefits that help low-income households buy the food they need for good health. The
goal of the program is to promote the general welfare and to safeguard the health and well being of the population through the
issuance of benefits to all eligible households. For most households, food stamps account for only a portion of their food budgets;
they must also use their own funds to buy food sufficient to last throughout the month.
Social Services Analysts in the parish offices are responsible for the determination of eligibility and the certification of eligible
households for food stamps. Electronic Benefits Transfer (EBT) performs the issuance of food stamp benefits to eligible
households. The employment and training activities, known as the Louisiana Job Employment Training (LaJET) Program, are
accomplished through the cooperative efforts of the Food Stamp Program, the OFS parish offices, local governing authorities in the
State and the Louisiana Department of Labor. The LaJET Program, in parishes where available, is a participant-centered effort to
provide employment services to selected food stamp recipients.
The USDA-Food and Nutrition Service base the amount of food stamp benefits that a household is eligible to receive on the Thrifty
Food Plan amounts that are established. They are current estimates of the actual costs for providing a household with nutritious but
inexpensive meals. These are the maximum amounts that a household with zero net monthly income can receive in food stamp
benefits each month. If a household meets all eligibility requirements, the amount of their food stamp benefits is dependent on both
the number of persons in the household and the net monthly income amount remaining after all allowable deductions have been
subtracted.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the State of Louisiana and fall into one of two groups: (1) those
with a current bank balance (savings and checking combined) under $2,001, or (2) those with a current bank balance (savings and
checking combined) under $3,001 who share their household with a person or persons age 60 and over, or with a person with a
disability (a child, your spouse, a parent, or yourself). For either group, you must also have an annual household income of less than
$13,284 if one person lives in the household; $17,808 if two people live in the household; $22,332 if three people live in the
household; $26,856 if four people live in the household; $31,380, if five people live in the household; $35,904 if six people live in the
household; $40,428 if seven people live in the household; or $44,952 if more than seven people live in the household.

Program Description
The National School Lunch and Breakfast Programs offer meal reimbursement on an income-based sliding scale for public and non-
public school students. Schools approved by the Board of Elementary and Secondary Education or residential childcare facilities
may apply for participation in the School Lunch and Breakfast Programs.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the State of Louisiana and a parent or primary caregiver
responsible for a child(ren) who attends school (high school or under). Those who qualify must also have an annual household
income before taxes that must not exceed $26,955 if two people live in the household; $33,874 if three people live in the household;
$40,793 if four people live in the household; $47,712 if five people live in the household; $54,631 if six people live in the household;
$61,550 if seven people live in the household; and $68,469 if eight people live in the household. For larger households, add $6,919 for
each additional person in the home.

Energy costs significantly influence the affordability of housing for everyone, often impacting the basic subsistence requirements of
a household. The Low Income Home Energy Assistance Program (LIHEAP) provides eligible households with assistance in meeting
their monthly utility bills. Priority is given those who may be elderly, disabled, have families with children, and/or have the highest
energy consumption.

Program Description
LaCHIP is a health insurance program designed to bring quality health care to currently uninsured children and youth up to the age
of 19 in Louisiana. Children can qualify for coverage under LaCHIP using higher income standards. LaCHIP provides Medicaid
coverage for doctor visits for primary care as well as preventive and emergency care, immunizations, prescription medications,
hospitalization, home health care and many other health services. LaCHIP provides health care coverage for the children of
Louisiana's working families with moderate and low incomes. Children must be under age 19 and not covered by health insurance.
Family income cannot be more than 200 percent of the Federal poverty level (about $3,067 monthly for a family of four or $36,800
annually). There are no enrollment fees, no premiums, no co-payments and no deductibles. NOTE: Even if your total income is more
than these amounts, your children may still qualify because you can receive these deductions: Earned Income (we deduct $90 for
each employed income unit member); Childcare Payments (we deduct $175 per month per child age two or older), or $200 per
month per child under age two; Child Support Received (we disregard up to $50); Child Support Payments to someone outside the
home (we deduct the amount actually paid up to the amount of the court order.) Children enrolled in LaCHIP will maintain their
eligibility for 12 continuous months no matter how much their family's income increases during this period. This is being done to
ensure children receive initial and follow-up care. A renewal of coverage is done after each 12-month period. Find out today whether

In order to qualify for this benefit program, you must be a resident of Louisiana, under 19 years of age, not covered by health
insurance (including Medicaid), a US national, citizen, legal alien, or permanent resident, and you must have an annual household
income before taxes of less than $36,432 if two people live in the household; $45,792 if three people live in the household; $55,152 if
four people live in the household; $64,512 if five people live in the household; $73,872 if six people live in the household; $83,232 if
seven people live in the household; $92,592 if eight people live in the household; and $101,952 if more than eight people live in the
household. For larger households, add $9,360 for each additional person in the home.  Depending on your income level, you may

Program Description
The WIC Program provides supplemental foods, nutrition education and referrals to health care, at no cost, to low-income pregnant,
breastfeeding and postpartum women, infants, and children up to age 5 who are determined to be at nutritional risk. To be fully
eligible for the WIC Program, applicants must be determined by a health professional, at no cost, to be at nutritional risk (i.e., have
certain medical-based or diet-based risk conditions).
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the State of Louisiana, pregnant, breastfeeding and
postpartum woman, infant or child up to 5 years of age and: (1) are individually determined by a health professional to be at nutrition
risk; and, (2) meet an income standard, or are determined automatically income eligible. A person who participates or has family
members who participate in certain other benefit programs, such as the Food Stamp Program, Medicaid or Temporary Assistance
for Needy Families, automatically meets the income eligibility requirement.

Program Description
The Special Milk Program (SMP) provides milk to children in schools and childcare institutions that do not participate in other
Federal child nutrition meal service programs. The program reimburses schools for the milk they serve.

Schools in the National School Lunch or School Breakfast Programs may also participate in the SMP to provide milk to children in
half-day pre-kindergarten and kindergarten programs where children do not have access to the school meal programs.

The Special Milk Program began in 1955 and is administered at the Federal level by the U.S. Department of Agriculture through its
Food and Nutrition Service, formerly the Food and Consumer Service.

General Program Requirements
Any child in a school or institution that participates in the Special Milk Program can get milk. Schools may elect to offer free milk to
low-income children. In order to qualify for this benefit program, you must be a resident of the State of Massachusetts, your
household's annual income before taxes must not exceed $20,036 if one person lives in the household; $26,955 if two people live in

Program Description
The Head Start program (for children ages 3-5) and Early Head Start program (for pregnant women, infants, and toddlers) promote
school readiness for children in low-income families by providing comprehensive educational, health, nutritional, and social
services. Parents play a large role in the programs, both as primary educators of their children and as participants in administering
the programs locally. Both programs provide pre-literacy and literacy experiences in a multi-cultural environment. Parents are also
provided social services, including assistance with childcare. Services are also available to migrant and seasonal farm worker
families.
General Program Requirements
In order to qualify for this benefit program, you must be a resident of the State of Louisiana, you must be a parent or primary
caregiver responsible for a child who is too young for public school and your household's annual income before taxes must not
exceed $10,830 if one person lives in the household; $14,570 if two people live in the household; $18,310 if three people live in the
household; $22,050 if four people live in the household; $25,790 if five people live in the household; $29,530 if six people live in the
household; $33,270 if seven people live in the household; $37,010 if eight people live in the household; and $40,750 if more than eight
people live in the household. For larger households, add $3,740 for each additional person in the home.

If you do not meet these criteria, you may still qualify if you are a U.S. national, citizen or permanent resident whose financial status
is low income or very low income, who is under-employed, unemployed or about to become unemployed, facing pregnancy, less
than 19 years of age yourself, or the parent or primary caregiver for children under the age of 19 years. Other qualifications may

Program Description
The Louisiana Medicaid Program is a Federally funded program, which provides health coverage to Louisiana residents, within
certain groups, based on income and resource limits set by Congress. Medicaid provides a complete package of medical coverage
for those who qualify. Generally, eligible recipients of benefits include low income families with children, low-income elderly, and
persons with disabilities. Persons who are entitled to Medicare Part A and/or Part B are also eligible. Mandatory medical services
included under Medicaid are: inpatient and outpatient hospital services; physician services; laboratory and x-ray services; long-term
care facilities (nursing homes); family planning; services for early periodic screening, diagnosis and treatment (EPSDT) of those
under age 21. To receive benefits, you must complete an application. We will make a decision and you will be notified within 45 days
(with some exceptions) after you apply. If your application is based on disability, it may take up to 90 days. Coverage can start as
early as 3 months before the month you apply. This is why you should get this form to us as soon as you can.

resident, or legal alien, in need of health care/insurance assistance, whose financial situation would be characterized as low

Program Description
The Family Independence Temporary Assistance Program (FITAP) provides cash assistance to families with children when the
financial resources of the family are insufficient to meet subsistence needs. The overall goal of FITAP is to decrease the long-term
dependency on welfare assistance by promoting job preparation and work. Public assistance is an opportunity to become
independent after a financial crisis.
The Social Security Act as amended by the Personal Responsibility and Work Opportunity Reconciliation Act of 1996 authorizes the
Family Independence Temporary Assistance Program. The Department of Health and Human Services is the Federal agency
responsible for its administration. The program is administered at the state level by the Department of Social Services, Office of
Family Support. Social Services Analysts in the parish offices throughout the state are responsible for processing applications for
FITAP, as well as completing re-determinations of eligibility. Electronic Benefits Transfer (EBT) performs the issuance of FITAP
benefits to eligible households.

In order to qualify for this benefit program, you must be a resident of Louisiana, either pregnant or responsible for a child under 19
years of age, a US national, citizen, legal alien, or permanent resident, have low or very low income, and be either under-employed
